This survey will ask you to provide us with some personal data to enable the Youth Sport Trust to manage your potential attendance at the Youth Sport Trust Young Changemaker Awards, sponsored by the Lego Group, in London on Wednesday 20 November 2024.

The Youth Sport Trust will be the data controller of the personal information you provide through this survey. This will be processed in accordance with our privacy notice which can be accessed here. By submitting any personal data to us, you are agreeing to the use of this information as set out above and in our privacy notice.

Photographs and/or video footage may be taken of delegates during the Youth Sport Trust Young Changemaker Awards by, or on behalf of the Youth Sport Trust, which may be featured in promotional literature (online/printed) and media activity (social media/website/national media coverage) about this and future events. The Youth Sport Trust may also use the photographs or video footage after the event, in promotional literature or media activity about their work. We have a legitimate interest to process images for these purposes. The Youth Sport Trust is funded by grants, donations and sponsorship, so photographs and videos are an important source of evidence of our work.
