The Youth Sport Trust (YST) are looking to gather some feedback from parents/carers who have watched the Active Families film, used some of the ideas, and as a result have been active with their children during the summer holidays.

As a family you may have also been given an Active Family pack (including equipment and cards) to use alongside the activities on the film. If you are happy to be contacted by a member of the YST to ask you a few questions about the impact of the film and/or activity pack please confirm so in the final part of the registration form.

As part of the registration process, we will need to collect some personal data about you and your child(ren) (if they will be taking part in any of the Active Family sessions with you). This information may be shared with session delivery partners, where required. We also need to collect some information for the purposes of monitoring diversity and equality of opportunity.

The Youth Sport Trust will be the data controller of the personal information you provide through this survey. Our External Privacy Notice, which can be accessed here includes full details of how we look after this data, information regarding your individual rights and how you can contact us. Please note that all personal data provided by you in this registration survey will be permanently deleted or anonymised within three months of the event.
